You are about to start the registered student form for CHEK ABC's distributed learning program. We will guide you through this process step by step. If you need to navigate back and forth between pages on the form please be sure to use the "previous" and "next" buttons at the bottom of the pages so that any information entered remains saved.

Please include the following with your application. These documents are necessary for the registry process: Your child will not be considered for registered until we receive these documents.

• • Student Birth Certificate or Passport
• • Copy of BC health card both sides
• • Records Request Form printed and signed
• • Immigration documents (if applicable, please include parents work permit and birth certificate/passport)

To enroll a student, click Add Student, complete the form, then click on Add Student to save. The student will appear below. Add one form per student.

Repeat this for each student you are enrolling.

• • You can add up to 5 Student Entries

12 files allowed with a max file size of 2MB each. If your documents exceed the limit, please email all your documents to admissions@chekabc.ca

There is no fee to register your children with CHEK ABC prior to Sept 30th.

After October 1st, we receive no funding for registered students so a charge of $50/student registration fee will be applied to offset administrative costs.
