You are about to start the Cross Enrollment form for CHEK ABC's online learning program. We will guide you through this process step by step.
Please include the following with your application. These documents are necessary for the cross-enrollment process: Your child will not be considered for cross- enrollment until we receive these documents.
If there are any files that you would like to submit with this online application, there will be a space to upload these files at the end of the application.
If you student withdraws from the course after it has been activated, there will be a $50 withdraw fee.

If you selected to pay your tuition by e-Transfer, that payment is due within 7 days of submitting this application. If payment is not received within 7 days your application will be withdrawn.
Please ensure that you have filled out the information accurately, and submitted all necessary documentation. We will contact you to confirm enrollment.
